Case
Management
Seniors
(and family members) are supplied with a
better understanding of COA services,
are referred for other services (COA
programs as well as other service
organization programs) for which they
are eligible, and/or are assisted in
finding the resources to meet their
needs. Case managers assist with Medicaid and
Medicare questions as well as many other
issues facing our senior population. Case managers are available
to identify needs, develop and monitor
service plans to assist in meeting the
identified needs, and to advocate for
and communicate with other agencies when
necessary for additional services.
